Welcome to My Wellness LLC (“My Wellness,” “we,” “our,” or “us”). By accessing or using our services, purchasing products, or receiving treatment, you agree to the following Terms & Conditions. Please read them carefully.
1. Services Provided
My Wellness offers wellness-related services including, but not limited to:
- IV hydration
- Weight-loss injections
- Peptide therapies
- Vitamin and wellness injections
- Consultations and follow-up services
All services are provided under medical oversight by a licensed physician/medical director.
2. Eligibility
You must be at least 18 years old to receive services unless a parent or guardian provides written consent.
My Wellness reserves the right to decline services for safety, medical, or professional reasons.
3. Medical Information & Consents
Clients must provide complete and accurate medical information. You agree to:
- Disclose your medical history truthfully
- Inform us of changes in medications or health conditions
- Complete all required consent forms before treatment
My Wellness is not responsible for adverse outcomes resulting from withheld or inaccurate information.
4. Payments
We accept:
- Credit and debit cards (processed through Stripe)
- ACH/bank payments
- Venmo Business payments
- Cash
Beginning January 2026, a small processing fee will apply to all card and Venmo payments.
Payment is due at the time of service unless otherwise arranged.
5. Appointment Policy
Appointments cannot be scheduled through the website.
All appointments must be arranged directly through the clinic via:
- Call
- Text
- Direct message (Facebook, Instagram, or other active platforms)
Appointment request times are not confirmed until the clinic has verified them.
Late arrivals may reduce treatment time or require rescheduling.
6. Cancellation & No-Show Policy
Cancellations must follow the clinic’s current Refund & Cancellation Policy.
Repeated no-shows may result in limited scheduling availability.
7. Product Sales
All medication (including peptides, injectables, and prescription therapies) is dispensed only under physician-approved standing orders.
Medication sales are final once products leave the clinic or are ordered specifically for a client.
8. No Medical Advice
Information shared on our website, social media, or in general conversation is for educational purposes only and does not replace professional medical advice, diagnosis, or treatment.
9. Liability
You agree that My Wellness, its staff, and its medical director are not responsible for:
- Reactions due to undisclosed medical conditions
- Improper use of products outside clinic instructions
- Failure to follow written or verbal guidance
10. Communication Consent
By contacting My Wellness, you consent to communication through:
- Phone
- Text message
- EMR messaging systems (Aesthetic Record or WeTreat)
You may update your communication preferences at any time.
11. Data & Privacy
Your information is handled according to our Privacy Policy.
We do not sell personal or medical information.
12. FDA Status of Certain Products
Some peptides and wellness products offered at My Wellness are not currently approved by the U.S. Food and Drug Administration (FDA). These therapies may be used off-label or as part of emerging research-based practices under physician supervision.
By receiving these services, you acknowledge that:
- Certain peptide therapies may be considered investigational or experimental.
- These products are not FDA approved for weight loss or general wellness uses.
- Risks, benefits, and alternatives are explained during the consent process.
- You will not hold My Wellness, its staff, or its medical director liable for outcomes associated with non-FDA-approved or off-label therapies.
My Wellness will always disclose when a product is not FDA approved and only provides such treatments under physician-approved standing orders.

We may update these Terms at any time. The most current version will always be posted on this page.
